شکل دارويي: شربت

موارد مصرف: مفيد در درمان سنگ کليه و مثانه و کيسه صفرا؛ کبد چرب؛ کاهش رطوبات مغز و ريه و کبد؛ تجفيف (خشکاندن) رطوبات زايد و عفونت‌ها

توضيحات: شربت عنصلي يکي از انواع سکنجبين‌ها مي‌باشد که کار آنها تقطيع و تحليل رطوبات است و موجب از بين رفتن اخلاط بلغمي و لزج در بدن مي‌شوند. به تبع آن با از بين رفتن رطوبات زايد، انسدادها برطرف شده و بيماري‌هاي ناشي از خلط غليظ و لزج و سده نيز برطرف مي‌شود.

با توجه به متون طب و داروسازي سنتي ايران، پياز عنصل (که نام ديگر آن اسقيل است) جهت بيشتر امراض رطوبي سر مانند ليثرغس (لتارژي يا کاهش سطح هوشياري همراه با خواب آلودگي و بي‌تفاوتي)، صرع، ماليخوليا، سردرد سرد، سبات (استوپور يا کاهش سطح هوشياري)، دوار (سرگيجه حقيقي)، سدر، اختلاج (پرش عضلاني)، استرخا، فلج، لقوه (فلج صورت)، و نسيان، نافع است و بينايي را تقويت کرده و منع نزول آب در چشم (آب مرواريد يا کاتاراکت) و درد گوش مي‌نمايد و لثه سست را مستحکم و حلق را تقويت مي‌کند و جهت بدبويي دهان و آسم بلغمي و تنگي نفس و سرفه کهنه و گرفتگي صدا و استفراغ همراه با خون و تقويت معده بر هضم غذا و تنقيه آن از رطوبات ناسالم مفيد است و نافع استسقا و يرقان و صلابت طحال و دل پيچه و مدر قوي ادرار مي‌باشد.

سرکه و شربت آن شکننده سنگ گرده و مثانه و نافع عسرالبول (دشواري در ادرار کردن) و احتباس حيض و مسقط جنين و نافع اختناق رحم و درد مفاصل و عرق‌النسا (درد سياتيک) و قروح ظاهري و درد عصب و داءالثعلب و جذام و منقي اعضا از اخلاط غليظ لزج به جهت جلا و تفتيح و جاذب فضول از عمق بدن و مسهل اخلاط غليظ است.

اجزاء فراورده: پياز عنصل (اسقيل)، سرکه انگوري، عسل مصفي

آثار دارو از ديد طب سنتي: جالي و مفتح و ملطف کيموسات غليظ و جاذب فضول از عمق بدن و ملطف و محلل و مسهل اخلاط غليظ به ويژه بلغم لزج، مره سودا و سوداي ناشي از خلط بلغم

مزاج کلي دارو: گرم و خشک

منع مصرف: انواع آريتمي‌هاي قلبي چون آسيستول، بلوک‌هاي دهليزي بطني درجه دو و سه، تاکي‌کاردي بطني و در نارسايي احتقاني قلب که همزمان از داروي ديگوکسين نيز استفاده مي‌شود؛ هيپوکالمي؛ زخم و التهاب حاد معده و روده

درصورت بروز حساسيت نسبت به اين دارو بايد قطع شده و بيمار به پزشک ارجاع داده شود.

عوارض جانبي: گرمي و گرگرفتگي؛ سردرد؛ حالت تهوع؛ سوزش معده در اثر ترشي سرکه؛ درد شکمي؛ و لينت مزاج

شربت عنصلي به علت وجود عسل در برخي موارد مي‌تواند منجر به افزايش قند خون شود.

دستور مصرف:

بزرگسالان: نيم‌ساعت پيش از صبحانه 20 سي‌سي(معادل دو قاشق غذاخوري) از شربت با يک استکان آب گرم حل شده و ميل شود. در صورت کثرت ماده، با نظر پزشک، شب پيش از خواب نيز مصرف مي‌شود.

کودکان: بر حسب وزن، دوز دارو را مي‌توان تا يک هشتم دوز بزرگسالان کاهش داد.

در برخي موارد براي مثال در ورم لوزه‌ها مي‌توان از غرغره شربت عنصلي استفاده کرد.

تداخلات دارويي: جذب دارو در حالت ناشتا بيشتر است؛ در عين حال مصرف بعد از غذا هم با توصيه طبيب معالج با ملاحظات مربوط به مزاج و بيماري قابل اجراست.

فراورده‌هاي حاوي املاح آهن در مدت زمان مصرف سکنجبين عنصلي تجويز نشوند.

مصرف همزمان با داروهاي ضد انعقاد مثل کلوپيدوگرل (Plavix)، هپارين، و تيکلوپيدين با احتياط و مشاوره طبيب و داروساز صورت پذيرد. اين دارو ممکن است با وارفارين تداخل داشته باشد و رقّت خون را افزايش دهد.

در صورت مصرف همزمان داروهايي مثل بتابلاکرها، و بلوک کننده‌هاي کانال کلسيم احتياط و تنظيم دوز توسط طبيب و داروساز صورت پذيرد.

مصرف در حاملگي و شيردهي: در دوران حاملگي و شيردهي منع مصرف دارد و بهتر است حتي الامکان براي اطفال زير سه سال نيز استفاده نشود و داروهاي جايگزين به‌کار رود.

نکات قابل توصيه: اين شربت در فصل گرم، شهرهاي گرم، گرم‌مزاجان، و جوانان با آب سرد ميل شود و در فصل سرد، شهرهاي سردسير، سردمزاجان، و پيران با آب گرم مصرف شود. در صورتي که بيمار از داروهاي رقيق کننده خون استفاده مي‌کند با احتياط مصرف شود يا با عرق کاسني-شاهتره همراه شود. اگر بيماري دچار خشکي مزاج است،‌ شربت عنصلي با آب سرد تجويز شود تا گرما، زياد نشده و خشکي تشديد نشود. در صورت ابتلا به زخم يا التهاب مزمن معده بهتر است دارو با احتياط و بعد از غذا تجويز شود.

Onsoly syrup

Drug form: Syrup

Indications: useful for treating kidney, bladder, and gall bladder stones; fatty liver; decreasing excess moisture of brain, lung, and liver; and desiccation of excess and infectious humors

Comments: Onsoly syrup is a type of oxymels. Oxymels are fragmenting and dissolving agents which can eliminate phlegmatic and viscous humor; therefore, after removal of these excess humors, blocked ducts and pores will be unclogged and diseases due to thick and viscous humors will be improved.

Squill bulb is useful for treating most of the brain diseases related to excess moisture such as lethargy, epilepsy, melancholia, cold headache, stupor, vertigo, dizziness, muscle tremor, paralysis, Bell’s palsy, and amnesia. This herb is effective in reinforcing eye sight; prevention of cataract, and othalgia; and strengthening of gum, and stomach. Moreover, it is useful for treating halitosis, phlegmatic asthma, dyspnea, chronic cough, hoarseness, hematemesis, dyspepsia, dropsy, icterus, spleen sclerosis, and abdominal cramps. Squill is a potent diuretic and a stomachic agent.

Squill vinegar and its syrup are abortive drugs and lithotripter agents for bladder and kidney stone and are effective in treating difficult urination, amenorrhea, hysteria, sciatica, joint pain, external ulcers, neuralgia, alopecia, and leprosy. They are detergent and unclogging agents, and absorbent of pathogenic and excess materials from the distant spaces and purgative agents for thick humors; therefore, they can cleanse thick humors from body organs.

Ingredients: Scilla maritima, vinegar, honey

Traditional mechanisms of action: A detergent and unclogging drug; rarefying of thick chymes and absorbent of pathogenic and excess materials from the distant spaces; effective in thinning down and dissolving and purgation of the viscous phlegmatic humors and black bile.

Drug temperament: hot and dry

Contraindications: cardiac arrhythmias such as asystole, second and third degree atrioventricular blocks, ventricular tachycardia, and congestive heart failure treated by digoxin, hypokalemia, acute GI inflammation and ulcer

If an allergic reaction to drug has occurred, it should be stopped and patient should refer to the physician.

Side effects: hotness sensation, flushing, headache, nausea, heart burn, abdominal pain, and looseness of the bowels. An increase of blood glucose is occasionally observed.

Administration and dosage:

Adults: 20cc (2 tablespoon) of syrup dissolved in 1 teacup boiling water taken orally half an hour before breakfast. If necessary, the drug can also be prescribed at bedtime under the physician direction.

Children: The dosage can be decreased to one-eighth of adult amount according to the child weight.

Sometimes, (eg. tonsillitis) the diluted syrup can be used in the form of gargling.

Interactions: Drug is better absorbed in fasting; however, consumption after meal is also recommended under physician consideration according to the patient temperament and type of disorder.

Iron preparations should not be taken in the duration of treatment with Onsoly syrup.

Simultaneous administration of this syrup with anticoagulants such as clopidogrel, heparin, ticlopidine; beta blockers; and calcium channel blockers should be carried out cautiously under physician and pharmacologist consideration. This drug may have interaction with warfarin and increases blood thinness.

Pregnancy and lactation: The drug is contraindicated in these two periods and it is better not to be administered to children under 3 years old and alternative drugs should be prescribed.

Recommendations: dissolve this syrup with cold water in warm seasons, warm regions, hot temperaments, and young adults; conversely, dissolve the syrup with hot water in cold seasons, cold regions, cold temperaments, and elderly. If anticoagulant or blood-thinning medications have been administered to the patient, use this syrup cautiously under physician direction and simultaneously with Cichorium intybus-Fumaria parviflora distilled water. If the patient has dry dystemperament, Onsoly syrup should be prescribed with cold water preventing from increase of dryness. The drug should be administered after meal and cautiously in patients with chronic peptic ulcer or inflammation.
